This page contains all the forms you'll need for camp this summer. You'll need Adobe Acrobat to download the

Summer Camp Enrollment Forms
You can either sign up online or print a hard copy of the application form, fill it out by hand, and fax or
mail it back to us. We begin accepting new enrollment for the 2012 summer season in January. Please fill out
our interest list form so we can notify you via email when the actual 2012 enrollment form becomes available.
Doctor's Health Form
Your child's doctor must complete and submit this form to us by June 1. (Even if your child has attended Aloha
Beach Camp in the past, we still need a new Doctor's Health Form this year.)
Code of Conduct Form
This form must be completed and signed by you and your child and returned to us by June 1. (Even if your child
has attended Aloha Beach Camp in the past we still need a new Code of Conduct Form this year.)
Transportation Sign Up Form
Family Handbook
The 2012 Family Handbook answers the most frequently asked questions about camp such as what to wear
and bring, transportation info, camp policies and other important details. Please review the Family Handbook
with your child so you both know what to expect this summer.
Employment Forms
If you want to apply for a summer job with us, you can access the online Employment Application from this
page. If you have already completed an Employment Application, we need a few additional pieces of
paperwork to complete your employment inquiry including two Employment Reference Forms and one
Reference Check Release Form.
